When you begin our personal injury intake process, you should be ready to tell us about your incident. Whether it was a car accident, slip and fall or dog bite, we want to hear the all the details and specifics. Be descriptive but keep the information relevant. This will help us to better assess your claim.

Specific Employer Information
For everyone;
-Please provide a doctors note from either of your providers that states the recommended time off to recover.
If you do not have a doctor's note from your hospital/urgent care visit, chiropractor, or primary care provider, we will not be able to include wage loss in your claim.
You may also request from your employer a detailed letter that explains the time you missed work and the amount lost as a result in substitution of a doctors note.
IF YOU DO NOT HAVE EITHER OF THESE DOCUMENTS, WE CANNOT INCLUDE YOUR WAGE LOSS.
For hourly employees;
-Please provide us paystubs from 4 months prior and 4 months after the collision.
-Please provide us Time Cards showing the time that was lost (if applicable).
For business owners;
-Please provide us profit and loss statements from 4 months prior and 4 months after the collision.
-If you had to hire other employees to complete the jobs you were initially assigned, please provide proof of employment and loss letter discussing the situation.
For contractors;
-Please provide us payment statements and/or invoices from 4 months prior and 4 months after the collision (if applicable).
